About Future Planning & Support
Our service aims to support ALL Young People aged between 17-25 years transitioning from Out of Home Care (OOHC) and/or those who have been in Statutory Care, Guardianship and/or Kinship Placement for a cumulative period of at least 12 months.
This service will target all Cultural diversity of young people, young parents, and those who have lived in residential care.
The pilot does not replace leaving care planning and aftercare support currently provided by PSP funded Service Providers and/or OOHC Case Managers. This continues on to be delivered as part of day-to-day support and case management in accordance with statutory requirements for Young People leaving care.
