Home based Occupational Therapy and other Allied Health Services i.e. physiotherapy for adult persons and frail Aged Persons (65+ years) living at home or in Residential Care. The focus of the service is to enhance independence and safety at home, enabling frail Aged Persons to stay as long as possible and as they wish in their own homes, safely. Home Safety Assessments, individualized therapy i.e. exercises and the practising or re-learning of functional skills of daily living i.e. personal care, domestic chores, community access and leisure. Sourcing of Aids & Equipment are conducted as well as application for funding assistance.
